Extractions and Space Maintenance

Pediatric Dental Office Serving the St. Louis Community

At Little Pearls Pediatric Dentistry, we understand that the extraction of a tooth can be a significant event in a child’s dental journey. While our primary goal is always to preserve natural teeth whenever possible. If this is not an option, we are able to treat most extractions in the office and will consider the need for space maintenance to ensure a healthy bite and alignment as your child continues to grow.

When Are Extractions Necessary?

Extractions may be necessary due to several reasons, including:

  • Severe Decay: When a tooth is non-restorable due to the extent of decay, removing it is indicated to prevent worsening pain and infection.
  • Overcrowding: Sometimes, teeth need to be removed to prepare for orthodontic treatment or assist in correcting the path of eruption of permanent teeth.
  • Dental Trauma: If a tooth is broken or damaged beyond repair due to injury, extraction may be indicated.
  • Primary Tooth Retention: Occasionally, primary (baby) teeth do not fall out naturally and hinder the eruption of permanent teeth, requiring extraction to facilitate proper dental alignment.

The Extraction Process

The process of tooth extraction at Little Pearls Pediatric Dentistry is performed with the utmost care and precision, ensuring your child’s comfort and safety throughout:

  1. Pre-Procedure Assessment: Dr. Mary Margaret will thoroughly examine your child’s teeth and will use X-Rays to confirm lack of restorability and discuss any anticipated complications.
  2. Anesthesia and Sedation Options: To ensure a pain-free experience, appropriate anesthesia and, if necessary, sedation options will be provided based on your child’s needs and the complexity of the extraction. Oftentimes, nitrous oxide inhalation combined with adequate local anesthesia is sufficient.
  3. The Extraction: The tooth is carefully removed and firm pressure applied to stop bleeding and promote healing.

Extractions and Space Maintenance

Importance of Space Maintenance

Following an extraction, especially of a primary tooth, it is crucial to consider the need for space maintenance as well as the patient’s tolerance for future removal of that space maintainer in-office. Space maintainers are appliances designed to hold the space left by the extracted tooth open until the permanent tooth is ready to come in. This prevents neighboring teeth from shifting into the gap, which may cause future alignment issues and affect the development of your child’s permanent teeth.

Space Maintainers

Depending on your child’s stage of development and bite as well as ability to tolerate future removal, a space maintainer may be recommended following extraction. Oftentimes, this may be made chairside the same visit as the extraction. In some circumstances where a tooth has been removed on both sides of a child’s mouth, we may take an impression to create a bilateral appliance from a dental lab, which would be cemented during a later visit.

Aftercare for Extractions and Space Maintainers

Proper aftercare is essential for healing and includes:

  • Post-Extraction Care: We provide detailed instructions on how to care for your child’s mouth after an extraction, including guidelines on eating, brushing, and pain management.
  • Space Maintainer Care: If a space maintainer is recommended, regular check-ups are important to monitor the progress of the permanent tooth erupting to determine best timing for removal.

FAQs

1. Why might my child need an extraction?
A tooth may need removal if it is severely decayed, infected, fractured, or blocking proper eruption or orthodontic movement.

2. Will the extraction be painful?
The area is numbed to keep children comfortable. Mild soreness afterward is normal and managed with simple home care.

3. What is a space maintainer and why is it important?
It is a small appliance that preserves room for the future permanent tooth after early loss of a baby tooth, helping prevent crowding.

4. How long does a space maintainer stay in?
Until the permanent tooth begins to erupt or the dentist determines it is no longer needed.

5. How do we care for the area and the appliance?
Follow the post-op instructions, brush gently near the site, avoid sticky or hard foods, and attend follow-up checks to ensure proper fit.
